A follow-up to a dental trauma case

Sometimes, a dental trauma story doesn’t end after the emergency treatment.
Some injured teeth require years of monitoring and follow-up before the next stage of treatment becomes necessary. This is exactly what happened with one of our patients, whom we first met following a traumatic fall several years ago.
At the time of the accident, her upper right central incisor was fractured and the pulp was involved. The tooth was stabilised and restored as part of the emergency management, while the condition of the injured teeth was closely monitored.
We knew from the beginning that the injured tooth might eventually require root canal treatment.
But sometimes, dentistry is not about doing everything at once.
It is about monitoring, timing and doing the right treatment when the tooth needs it.
A Few Years Later, She Came Back
As the years went by, our patient continued with her life.
Eventually, the time came to continue the treatment that had been anticipated from the beginning.
She returned to White Density Dental Clinic and we proceeded with root canal treatment of the upper right central incisor.
The goal was to manage the tooth internally and preserve it as a functional part of her smile.
For a front tooth, however, there is another consideration beyond simply keeping the tooth healthy.
Its colour matters too.
When a Root Canal Treated Tooth Starts to Discolour

A few years after the root canal treatment, the tooth gradually became noticeably darker than the neighbouring teeth.
This can happen with root canal treated teeth. Because the tooth has undergone trauma and internal treatment, its colour can change over time.
For our patient, the difference in colour became increasingly noticeable because it was one of her front teeth.
She wanted to improve the appearance of the tooth while preserving as much of her natural tooth structure as possible.
After assessing the tooth, we discussed the available options with her.
One of the options was internal bleaching.
What Is Internal Bleaching?

Unlike conventional teeth whitening, which lightens teeth from the outside, internal bleaching is specifically used to lighten a discoloured tooth from within.
This can be particularly useful for a tooth that has darkened following trauma and root canal treatment.
The bleaching material is placed inside the tooth, allowing the discolouration to be addressed from its internal aspect.
The aim is not simply to make the tooth “white”.
It is to achieve a shade that looks natural and harmonious with the surrounding teeth.
From Trauma to a Natural-Looking Smile

After the internal bleaching treatment, the colour of the upper right central incisor improved significantly.
What makes us especially happy about this case is not just the final colour.
It is the journey behind it.
From an unexpected traumatic injury…
To emergency treatment…
To years of monitoring…
To completing the root canal treatment…
And eventually addressing the discolouration that appeared years later.
It reminds us that some dental cases are not completed in one appointment—or even one year.
Sometimes, we simply need to walk alongside our patients throughout the journey.
🦷 Why Long-Term Follow-Up Matters After Dental Trauma
Dental trauma can affect a tooth in ways that are not always immediately visible.
Even when a tooth appears comfortable after the initial emergency treatment, changes can develop later. This is why injured teeth may require regular clinical and radiographic monitoring over an extended period. Our original case notes specifically highlighted the need for timely follow-up, potentially for up to five years after significant trauma.
The important thing is not to assume that a tooth is “fine forever” simply because it does not hurt.
Regular follow-up allows us to identify changes early and decide when further treatment is appropriate.

Looking back at this case, I am especially grateful that our patient came back to continue her treatment with us.
Her journey reminds us that dental care is not always about achieving an immediate final result.
Sometimes, it is about being there for our patients at every stage—from the day an accident happens, through years of monitoring, and finally to the moment they are happy with their smile again.
